The combined procedure is under general anaesthesia. Power-assisted liposuction (PAL) is used to fat from the midsection. The fat is then purified and injected into the buttocks under ultrasound guidance, with placement kept above the gluteal muscle layer to avoid the deep venous plexus.

Swelling and are at their peak in the first two weeks and subside gradually over six to eight weeks. A garment is worn full-time for the first six weeks. Sitting directly on the buttocks should be for two to three weeks — a BBL pillow is used to redistribute pressure to the thighs. Most patients return to office work at two to three weeks; strenuous exercise is delayed until six to eight weeks.

The buttocks initially appear larger than the final result because of swelling and because surgeons over-graft to compensate for the 30–40% of fat that the body reabsorbs in the first three months. The final shape becomes apparent at three to six months once the surviving fat has stabilised.
